NOTES ON REGISTRATION:

The applicant should read the “Guidelines for Administration of the Class Licence for Offer of Telecommunications Services” before completing this registration form.
The email address of the regulatory contact person to be provided in this registration form shall mainly be used for communication with the applicant.
The applicant shall retain a copy of the completed registration form to facilitate subsequent updates of information and/or notification of cessation of services.
The Office of the Communications Authority (“OFCA”) may require further information or clarification from the applicant. Upon receipt of all required information, OFCA will proceed to assess the application, and subject to the Communications Authority (“CA”)’s discretion, the registration may be approved and a confirmation of such approval will be issued to the successful applicant within 14 working days.

SUPPLEMENTARY NOTES ON THE PROVISION OF PERSONAL DATA:

The provision of personal data in this registration form is voluntary. If the applicant does not provide sufficient information, OFCA may not be able to process the registration.
The personal data provided by the applicant may be used by the CA to process the registration and compile the register and to perform its functions, or exercise its powers under the Telecommunications Ordinance (Cap. 106) (“TO”).
Under section 7D of the TO, the CA shall keep a register of registered class licensees for offer of telecommunications services and shall keep the register available for public inspection. The CA may publish the register on its website.
The personal data provided by the applicant may be disclosed to other government departments/agencies for verification purpose.
Personal data will be handled in accordance with the relevant provisions of the Personal Data (Privacy) Ordinance (Cap. 486).
The applicant has a right of request for access and correction with respect to her/his personal data. The right of access includes the right to obtain a copy of the personal data provided by the applicant in the registration form, though a charge may be levied on obtaining such information.
