I acknowledge that, despite that the CA will generally preserve the confidentiality of confidential information that I submitted in relation to this complaint, including the information relating to my identity, the CA is empowered under the CO to disclose the confidential information it obtained under specified circumstances including for the performance of its functions under the CO or in connection with legal proceedings.

I agree that the personal data provided in this complaint will be used for the purpose of considering, responding to and taking follow-up action upon complaints, investigations, if appropriate, and any enforcement action under the CO.
